Elm
Elm is a residential service for physical disability and learning disability. A purpose built bungalow to accommodate three people, which is spacious and homely. The accommodation comprises of three bedrooms with ensuite facility, a bathroom with a specialist bath and an open plan kitchen, lounge area.
Individuals accessing this service benefit from the relaxed atmosphere and low stimulus environment. Professionals linked to the service support rehabilitation and care programmes by offering physiotherapy, psychology and education pathways.
Person centred planning is key to achieving a realistic and achievable progression route for the individual, where their needs and wishes are explored and promoted in a way that encompasses their best interests, risk probability and wellbeing.
Above all, Elm aims to achieve a home from home environment, where the individuals using the service are happy and comfortable, the staff are welcoming and hospitable and visitors are encouraged to visit their relatives/friends and any time that is suitable for their loved one.
