Making a referral is a simple, after an initial enquiry by phone, fax or email you will be asked to forward all available reports for consideration by the clinical team. The next stage is often a visit to our premises where we can discuss how we can care and support potential service users. We will then complete a clinical assessment and report back with our care costings. If required short or long term transition planning and familiarisation is commenced before the admission stage. Post admission reports, assessments and reviews follow admission.
Emergency referral advice can be obtained by speaking to the duty team leader using the main number, who will then pass details to the on call manager to make contact.
